New Antimicrobials Celebrated as a 'Major Shift' in Addressing Antibiotic-Resistant Gonorrhoea

The first new treatments for gonorrhoea in many years are being hailed as a "major milestone" in the battle against superbug strains of the infection, according to scientists.

A Worldwide Challenge

Cases of gonorrhoea are escalating globally, with data suggesting in excess of 82 million new cases per year. Especially elevated rates are seen in Africa and countries within the WHO's Western Pacific region, which encompasses Mongolia and China to New Zealand. Within England, cases have hit a record high, while rates across Europe in 2023 were significantly elevated compared to the rates from 2014.

“The clearance of fresh medications for gonorrhoea is an significant and necessary development in the face of rising global incidence, the spread of superbugs and the highly restricted therapeutic options currently available.”

Health officials are increasingly worried about the increase in drug-resistant strains. The global health body has classified it as a "critical concern". A tracking program showed that resistance to key first-line drugs like ceftriaxone and cefixime jumped significantly between 2022 and 2024.

A Pair of Novel Treatment Options Gain Clearance

One new antibiotic, alternatively called Nuzolvence, was cleared by the American regulatory agency in mid-December for treating gonorrhoea. This STI can lead to significant complications, including the inability to conceive. Researchers anticipate that focused deployment of this new drug will help slow the development of resistance.

Gepotidacin, developed by the drugmaker GSK, also received approval in close succession. This treatment, which is also used to treat UTIs, was proven in research to be able to combat superbug versions of the gonorrhoea bacteria.

A Unique Partnership

Zoliflodacin was the result of a new, not-for-profit approach for antibiotic development. The charitable organization GARDP partnered with the pharmaceutical company its industry partner to develop it.

“This milestone represents a major breakthrough in the therapy of highly resistant gonorrhoea, which previously has been evolving faster than our drug pipeline.”

Testing Data and Worldwide Availability

According to data published in a prominent scientific publication, zoliflodacin successfully treated the vast majority of genital gonorrhoea infections. This puts it on an comparable level with the current standard treatment, which involves an injection and a pill. The trial involved over 900 volunteers from several countries including Belgium, the Netherlands, South Africa, Thailand and the US.

Under the terms of its development partnership, GARDP has the rights to make available and distribute the drug in many regions with limited resources.

Clinicians directly involved have shared positive views. Having a one-pill regimen like this is described as a "game-changer" for managing the epidemic. This is deemed essential to reduce the burden of the disease for individuals and to prevent the spread of extremely resistant gonorrhoea worldwide.
